Make sure all forms are filled out completely and signed by parent/guardian and/or health care provider as needed.  These forms must be filed with the school nurse before any medications can legally be provided to your child.

Bring forms and medications to school during the week of August 24th-27th (8am-2pm) or during the Open House on August 31st.
