Dear Crivitz School District Families, 

Welcome to the School Nurse/Health Services information page!

We, Cindy Kubicek, RN BSN and Salina Polomis, CNA/Health Room Assistant, are proud to be apart of this great team of professionals dedicated to helping your child(ren) learn, grow, and achieve their potential. Our role in the health services office is to make sure that your child's health needs are met, so they can focus their attention and energy on learning. With your help, we'd like to get to know the basics about your child's health history and any current health care needs your child may require. 

Please take a moment to completely and thoroughly fill in the health infomation sections on the online registration form. Also, utilize the FORMS area on the left side to print off Medication Forms (for prescription and over-the-counter medication needs during school), Emergency Action Plans (for those with a history of allergic reactions, asthma, diabetes and seizures). Have these forms and any medications (needed during school hours), turned in BEFORE the start of school. Please make sure the forms are signed by your child's physician, if needed, prior to dropping off forms and any medication(s). Prescription medication(s) cannot be administered without a physician's consent.

We are always available during Open House at the start of each school year to meet directly with parents. Questions regarding immunization requirements? 

Before school begins, please make sure your child(ren) immunizations are up-to-date. State law requires schools to be compliant with certain age/grade requirements. Please visit https://www.dhs.wisconsin.gov/immunization/reqs.htm, for more information. 

If you have health, religious or personal reasons why your child should NOT be immunized, an official waiver must be filed with the school.  See the Student Immunization Record/Waiver on the left of this screen under FORMS. 

If your child is entering Developmental Kindergarten, Kindergarten (5K) or the 6th Grade this fall, please note the vaccination requirements needed in these grades.
